Overview 

Avenues Australia is a for-purpose organisation founded in 2020 committed to enhancing the coordination, management, and administration of health and social services for individuals living with complex health conditions, disabilities, long-term mental health conditions and social circumstances. Our navigational services address the fragmented nature of available health and social services by providing individualised assessments, service planning, case management, coaching, coordination and financial administration of appropriate support services

Through our Mental Health and Psychosocial, Disability and Aged-Care services, we leverage system navigators to ensure our clients receive the right services at the right time and at the right place in a trauma informed, client-directed, collaborative and safe manner. We provide the following services:

We provide NDIS and Aged Care navigation, and administrative services These include:

  • Mental Health and Psychosocial Services: Psychosocial Recovery Coaching
  • Disability Services: Support Coordination and Plan Management
  • Aged Care: Care Coordination

Support Coordinator / Recovery Coach 

The NDIS Support Coordinator / Recovery Coach is a dynamic hybrid role working both as a Support Coordinator and Recovery Coach. 

A Support Coordinator will be able to build a sound understanding of the complexity of the disability and community sectors within the context of the NDIS quickly, as well as knowledge of various services and supports in relevant fields to make good recommendations to our NDIS clients to better meet their individual needs. As a Support Coordinator, not only will you support our clients and informal support system in their day-to-day lives, you will also have a proactive and outcome-led approach to help them build capacity, chart a path to independence and acquire new skills and coping strategies.

A Recovery Coach assists NDIS participants with Psychosocial disability to build capacity and resilience through strong and respectful relationships to live a full and contributing life. This support is designed to be able to maintain engagement through periods of increased support needs due to the episodic nature of the mental illness. Recovery coaches work collaboratively with participants, families, carers and other services to identify, plan, design and coordinate NDIS supports. 

Responsibilities

The Support Coordinator / Recovery Coach key responsibilities will have the following functions:

General

  • Support clients and carers to understand their NDIS plan, goals and objectives. 
  • Support clients to develop, implement, monitor a support approach and/or adjust a personal recovery plan
  • Build a recovery-enabling relationship based on hope.
  • Working closely with clients to identify their needs, personalise support and ensure they are receiving appropriate services.
  • Promoting and fostering independent decision-making, including choice and control.
  • Demonstrated skill in researching information and developing effective and innovative support options. 
  • Coordinating and liaising with service providers, reviewing goals and jointly determining new ones with the client.
  • Identifying early indications of functional decline or safe-guarding risks, designing an appropriate support approach and linking the person with appropriate allied health services. 
  • Working collaboratively with allied health service providers to identify and mitigate key risks, and support the person to balance dignity of risk, choice and control and safety. 
  • Requesting, scrutinising and interpreting and implementing functional capacity assessments from relevant allied health practitioners as well as behaviour support practitioners to determine the nature and type of funding and support required 
  • Working collaboratively with allied health practitioners to provide information for the development of functional capacity assessments through dissemination of information, coordination and liaison. 
  • Supporting allied health practitioners in implementing therapy strategies. 
  • Work collaboratively with physicians and specialist doctors to implement health supports.
  • Work collaboratively with other community service providers to support community and economic participation. 
  • Coordinate the supports in an individuals’ NDIS plan by: 

o   researching, coordinating and managing complex and intersecting supports to suit individual needs across multiple providers while ensuring client choice and control; and

o   working with mainstream service providers including negotiating services to ensure service obligations are met. 

  • Coaching to build on knowledge, skills, and decision-making with the aim of greater choice, control and independence in managing their personal choices. 
  • Decide the budget for each support type and advise any relevant plan manager or service provider of the breakdown of funds. 
  • Liaise with plan managers and/or NDIS to establish the appropriate claim categories and attribute funds accordingly. 
  • Assist clients to prepare for plan reviews by helping them assess whether they achieved their goals and got value for money. Identify solutions to problems experienced in implementing their plan and to consider new goals. 
  • Assist clients to communicate with the NDIA when required to support their plan requirements. 
  • Resolve any crisis by linking participants with the appropriate services. 

Operational 

  • Ensure the completion of NDIS Participant Service Agreements and other appropriate documentation within the initial assessment as required. 
  • Maintain good quality documentation of clients. 
  • Monitor and report client outcomes to the NDIA through progress reports.
  • Actively participate in the development, implementation, monitoring, regular review and reporting of client programs with providers including determining risk and preparing assessments, review client records and understand individual needs, review client goals and measure individual progress and document outcomes. 
  • Promote Avenues Australia as an NDIS provider to participants and other stakeholders. 

Other Duties 

  • Conduct other duties as directed. 

Your Key CompetenciesCandidates must demonstrate the following competenciesIndustry: General industry knowledge on the NDIS, disability and mental health sectors.

Technical: General knowledge of best practice in Mental Health and disability practices.

IT Systems: Ability to understand and learn IT systems quickly to support the documentation and protection of essential and sensitive information. 

Interpersonal: Excellent person-centred communication skills.

Qualifications and Experience

  • A minimum of a Certificate 4 in Mental Health or Community Services or equivalent training or be working towards this qualification. 
  • Minimum two years’ experience in the NDIS or health industry or similar roles. 
  • Current First Aid and Mental Health First Aid credentials. 
  • Current Victorian Driver’s License and ability to undertake travel within the Melbourne Metro as required. 
  • Upon employment offer, a NDIS Workers Screening Check would need to be completed.
